Under Texas Health & Safety Code ยง181.105, therapists must provide clear instructions about how to request your health records and how to contact and file complaints with the state licensing authority.

To request copies of your health records, please send a request via email to Caitlin.Moya@MonarchTherapyPractice.com.
If you received treatment for couples therapy, there will be one file kept for all joint sessions. I must receive permission from all parties to release your records in full. If this is not possible, I will release a file in which all information regarding parties other than yourself is redacted.

Complaints against providers regulated under Texas law are handled by the relevant state agency. To file a complaint about our services, download and complete the BHEC complaint form and submit it per the instructions on the form (email or mail).

The Texas Behavioral Executive Council investigates and prosecutes professional misconduct committed by its licensees, including marriage and family therapists, professional counselors, psychologists, psychological associates, social workers, and licensed specialists in school psychology. Although not every complaint against or dispute with a licensee involves professional misconduct, the Executive Council will provide you with information about how to file a complaint. This notice is provided as required by Rule 884.31 of the Texas Behavioral Health Executive Council.
